Нерешенные вопросы этио­патогенеза преэклампсии как нейрогенного нейроиммунного явления перинатального периода

Проблема преэклампсии (ПЭ) – тяжелого гипертензивного протеин-зависимого осложнения второй половины беременности, являющегося ведущей причиной материнской и перинатальной заболеваемости и смертности во всем мире, сохраняющего нестабильную динамику, не теряет своей актуальности и оставляет широкий спектр нерешенных вопросов – от этиопатогенеза и поиска возможных маркеров до прогностической, диагностической и терапевтической тактики.
В представленном обзоре выдвигается концепция о рассмотрении ПЭ как явления стресс-дезадаптации перинатального периода, конфликта матери и плода, ассоциированного с нарушениями нейрокортикогенеза и формирования структур и функций высшей нервной деятельности, целостности и проницаемости гематоэнцефалического барьера (ГЭБ), что отражается в изменении уровня нейронспецифических белков (НСБ) в материнской сыворотке крови, наряду с процессом комплемент-опосредованного индуцированного материнскими агонистическими аутоантителами деструктивного гестационного эндотелиоза.
В обзор включены публикации, представленные в базах PubMed, Google Scholar и eLibrary по данной проблематике, опубликованные с 2016 по 2026 г.
Заключение. Тема статьи открывает новые направления изучения этиопатогенетических механизмов ПЭ как нейрогенного нейроиммунного явления, возможности поиска инновационных прогностически ценных диагностических и терапевтических мишеней, ранних предикторов неврологических осложнений у детей, рожденных от матерей с ПЭ, что может изменить как подходы к мониторингу и ведению беременности, так и понимание этиопатогенетической сущности ПЭ в целом.
