Роль предменструального дисфорического расстройства в формировании риска послеродовой депрессии: эпидемиологические и нейробиологические аспекты

Послеродовая депрессия остается одним из наиболее клинически значимых аффективных расстройств репродуктивного периода, оказывающим долговременное влияние на психическое здоровье женщины и развитие ребенка. Несмотря на выявление широкого спектра психосоциальных и психиатрических факторов риска, их индивидуальная прогностическая ценность остается ограниченной. В последние годы все большее внимание уделяется концепции гормональной чувствительности как ключевому механизму уязвимости к аффективным расстройствам, ассоциированным с репродуктивными переходами. В этом контексте предменструальное дисфорическое расстройство рассматривается как клинический фенотип атипичного нейробиологического ответа на физиологические колебания репродуктивных гормонов. В настоящем обзоре обобщены эпидемиологические, клинические и молекулярные данные, свидетельствующие о связи предменструального дисфорического расстройства с повышенным риском развития послеродовой депрессии. Рассматриваются результаты систематических обзоров, метаанализов и проспективных когортных исследований, демонстрирующих устойчивую ассоциацию между предменструальными аффективными нарушениями и послеродовой депрессией. Особое внимание уделено нейробиологическим механизмам пересечения двух состояний, включая нейростероидную модуляцию ГАМКергической передачи, пластичность ГАМК A-рецепторов, а также вклад серотонинергической системы и гипоталамо-гипофизарно-надпочечниковой оси. Отдельно обсуждаются экспериментальные и фармакологические данные, подтверждающие роль гормональной чувствительности как общего патогенетического звена, а также клинические импликации использования предменструального дисфорического расстройства в качестве маркера повышенного риска послеродовой депрессии.
Заключение. Представленные данные поддерживают концепцию репродуктивной гормональной чувствительности как объединяющего нейробиологического механизма, лежащего в основе предменструального дисфорического расстройства и послеродовой депрессии, и подчеркивают необходимость интеграции клинических, эпидемиологических и молекулярных подходов для более точного прогнозирования и профилактики послеродовой депрессии.
